This 12-month membership includes:

Up to five physician visits
Each visit may be scheduled for up to 30 minutes
Visits may be used flexibly throughout the membership year based on your needs

Visits may be used for ongoing functional medicine follow-up, lab or specialty-test review, treatment adjustments, medication or supplement management, or new health concerns.

Additional notes

Visits may be completed in person or virtually, when appropriate.
All five visits must be used within the 12-month membership period.
Unused visits do not roll over, transfer, or convert to credit.
Shorter focused visits may be scheduled when clinically appropriate and still count as one of the five included visits.
Laboratory testing, specialty testing, supplements, medications, procedures, and other products or services are not included.
Continuing Care is a 12-month commitment.
