When more than one eligible member of the same household enrolls in Continuing Care, additional household members receive a reduced rate.

Primary household member:
$1,575/year or $135/month

Each additional eligible household member:
$1,380/year or $115/month

Each person has their own membership and receives their own five annual visits.

Household pricing is available for spouses or domestic partners and dependent children living in the same household.

Interest-free payment plans may be available when needed.

A common payment arrangement is:

$200/month until the applicable balance is paid in full

Payment plans are separate from Continuing Care Membership and do not include additional visits or membership benefits.

We appreciate when our patients refer friends and family to Precision Functional Medicine.

When a referred patient completes the New Patient Starter:

- The new patient receives a $50 PFM credit

- The referring patient receives a $50 PFM credit

Referral credits may be applied toward eligible future PFM services.

Continuing Care is a 12-month membership.

The monthly payment option is a way to spread the annual membership cost over 12 payments and is not a month-to-month membership.

A few important details:

- Unused visits do not roll over into the next membership year

- Visits cannot be transferred to another person

- Shorter focused visits may be used when clinically appropriate and count as one of the five included visits

- Membership does not include unlimited visits or unlimited physician access

- Standard scheduling and cancellation policies continue to apply

- Early cancellation may be considered in limited circumstances and may require reconciliation of care already received at standard a la carte rates

The time you spend face-to-face with Dr. Coller is only one part of your care.

Depending on your case, physician time outside of the appointment may include:

- Reviewing your chart, prior treatment plan, and relevant records

- Reviewing laboratory and specialty-test results

- Preparing for the issues you plan to discuss

- Completing medical documentation and updating your care plan

- Ordering laboratory or specialty testing

- Prescribing medications and preparing supplement recommendations

- Reviewing incoming results and specialist recommendations

- Researching unusual or complex clinical questions when needed

- Responding to reasonable portal messages that do not require a full appointment

This ongoing physician work is part of both membership and a la carte care.
